Functional & Housing Assessments

Jordan conducts Functional Capacity Assessments (FCAs) and Housing and Support Needs Assessments to provide a detailed picture of an individual’s abilities and challenges. These assessments are often used to support NDIS funding requests and help determine the most appropriate level of care, housing, and support required to live as independently as possible.

Jordan works with clients to assess, prescribe, and implement assistive technology and home modifications that improve independence and safety. From mobility aids to bathroom alterations, each solution is personalised and aligned with the client’s needs, with full reporting and support for NDIS applications if required.
Therapy & Skill Development

Jordan offers tailored therapy to help clients build everyday skills and improve their quality of life. This includes task-specific training in daily activities like cooking, dressing, or budgeting, as well as retraining in mental health management, upper limb function, and cognitive skills. Therapy is guided by the client’s personal goals and delivered in familiar environments to maximise relevance and comfort.
Training & Safety
Equipping individuals and their support networks with skills to stay safe and confident.
Through Manual Handling Training, Jordan provides essential education to carers, support workers, and clients on safe movement techniques. This helps reduce injury risks and promotes confidence when providing or receiving physical assistance in daily tasks.
